Please read the following terms and conditions before you submit this enrolment form:

I understand that I can borrow a MAXIMUM of 4 resources at any given time.
I understand that I can borrow books and/or videos for a two week period.
I understand that if I have books and/or videos out for more than a month without renewing the resources I will be fined $1 per day and will be unable to borrow any other resources until the fine is paid.
I understand that if I have books and/or videos out for more than 3 months I will be sent a bill for their replacement costs.
I understand that if I damage or deface the resources in anyway I will be responsible for their replacement costs.
